Intake Process

Step 1: Referral

Connect with your primary care physician and have them fax a referral for the desired service.

Step 2: Phone Call

Once the referral is received, a therapist will contact you regarding scheduling.

Step 3: Evaluation

Receive a comprehensive evaluation to identify areas of need and create a treatment plan. Please bring the completed intake forms below to your evaluation.
